Co-Payments
We collect co-payments at the time of the appointment. Cash, credit, and checks are accepted for payment of services. In addition, our staff will make every effort to collect balance forwards at the time of your follow-up appointments.
Self-Pay Patients
- Initial Office Visit – $350.00*
- Follow-up Office Visits – $150.00*
- Any & All Test Fees Apply According to Medicare Pay Rate
