Additionally, some health plans require pre-certification and/or referral forms. Therefore, we will need a referral before we can provide any services.
In addition, we may need to obtain pre-certification from your insurance carrier before certain procedures and/or equipment may be ordered. Each insurance carrier has different coverage depending on the contract with that particular employer. Be familiar with your insurance coverage!
